Who is on my care team?

Good care management takes a village. Our Care Navigation program is a diverse team of clinical and non-clinical resources ready to help you every step of the way.

Nurses

The backbone of our program. Our nurse case managers work closely with you to manage your care, answer your questions, coordinate care and help you navigate the healthcare system.

Social workers

They remove obstacles for a living. Our social workers provide help when your health is impacted by economic or social conditions, like arranging free transportation to appointments or finding help paying for prescription medications.

Pharmacists

A pharmacist can help make sure your medication needs are being met with at the most reasonable cost, while also making sure all your medications work well together.

Medical directors

These individuals provide clinical expertise to our nurse care managers and make phone calls to your providers to help explain your benefits and our coverage policy.

    First time users will be asked to register with a few basic questions.
  3. Begin messaging with a care manager
    Once you’re enrolled in the program and registered, you can easily communicate with your care manager.
